Located at 15033 W Bell Rd, Suite 175, Surprise, AZ 85374, the Wellness Dental Surprise office provides comprehensive general, cosmetic, restorative, and implant dentistry in one Northwest Valley location. The office is open Monday through Thursday from 9:00 a.m. to 5:00 p.m. and Friday from 9:00 a.m. to 2:00 p.m., and can be reached at (623) 323-5857.

Dr. Jensen earned his dental degree at Columbia University, completed a hospital residency at Montefiore Medical Center, and received surgical implant training at NYU.
